Job Title: LPN, Long Term Care (LTC)
Job Summary:
The LPN in Long Term Care (LTC) is responsible for providing skilled nursing care to residents in a long-term care facility. This includes assessing residents' health needs, developing care plans, administering medications, and collaborating with other healthcare professionals to ensure the well-being of residents. The LPN also provides support and assistance to residents with activities of daily living and promotes a safe and comfortable environment for all residents.
Responsibilities and Duties:
- Conducting nursing assessments and developing individualized care plans for residents
- Administering medications and treatments according to physician orders
- Monitoring residents' health status and responding to changes in condition
- Collaborating with physicians, therapists, and other healthcare professionals to provide comprehensive care to residents
- Documenting residents' health information and maintaining accurate medical records
- Providing emotional support and encouragement to residents and their families
- Assisting residents with activities of daily living, such as bathing, dressing, and eating
- Adhering to all federal, state, and facility regulations and guidelines for nursing care
- Participating in staff meetings and continuing education opportunities to enhance nursing skills
Qualifications and Skills:
- Current LPN license in the state of practice
- Experience in long-term care or geriatric nursing preferred
- Excellent communication and interpersonal skills
- Strong attention to detail and organizational abilities
- Ability to work effectively in a team setting
- Compassionate and caring attitude towards residents
- Knowledge of nursing techniques and procedures
