Job title: Hospice/Medical Social Worker
Job Summary:
As a Hospice/Medical Social Worker, you will provide emotional support and counseling to patients and their families facing end-of-life care. You will assess the social and emotional needs of patients, develop care plans, and connect them with community resources. Your role will be crucial in providing compassionate care and improving the quality of life for those in hospice care.
Responsibilities and Duties:
1. Conduct psychosocial assessments of patients and their families.
2. Develop and implement care plans to address social and emotional needs.
3. Provide counseling and support to patients and families.
4. Coordinate with interdisciplinary team members to ensure comprehensive care.
5. Advocate for patient rights and facilitate communication between patients, families, and healthcare providers.
6. Maintain accurate and up-to-date documentation.
7. Collaborate with community resources to address social determinants of health.
8. Participate in interdisciplinary team meetings and case conferences.
9. Provide education and support to patients and families on end-of-life issues.
10. Evaluate and monitor the effectiveness of interventions.
Qualifications and Skills:
1. Master's degree in social work or related field.
2. Licensed Clinical Social Worker (LCSW) preferred.
3. Experience in hospice or healthcare setting.
4. Knowledge of medical social work practices and end-of-life care.
5. Excellent communication and interpersonal skills.
6. Ability to work effectively in a team environment.
7. Strong assessment and counseling skills.
8. Proficiency in electronic documentation systems.
9. Compassion and empathy for patients and families.
