Job title: Medical Social Worker
Job Summary:
As a Medical Social Worker, you will be responsible for providing emotional support, counseling, and advocacy for patients and their families in a healthcare setting. You will work closely with medical professionals to address the social and emotional factors that impact a patient's health and wellbeing.
Responsibilities and Duties:
1. Conduct psychosocial assessments of patients and their families to identify needs and develop care plans.
2. Provide counseling and support to patients and families dealing with medical issues, disabilities, or chronic illnesses.
3. Coordinate with healthcare teams to ensure a holistic approach to patient care.
4. Advocate for patients to access resources, services, and community support.
5. Collaborate with community agencies and organizations to facilitate referrals and access to resources.
6. Maintain accurate and up-to-date documentation of patient interactions and progress.
7. Provide education and support to patients regarding healthcare decisions and advance directives.
8. Participate in interdisciplinary team meetings to discuss patient cases and develop care plans.
9. Stay current on social work practices, ethics, and regulations related to healthcare.
Qualifications and Skills:
1. Master's degree in Social Work from an accredited program.
2. State licensure as a Clinical Social Worker (LCSW) or eligible for licensure.
3. Previous experience in a healthcare setting, preferably in a medical social work role.
4. Strong communication and interpersonal skills.
5. Ability to work effectively in a fast-paced and high-stress environment.
6. Knowledge of medical terminology and healthcare systems.
7. Ability to collaborate with multidisciplinary teams.
8. Empathy and compassion for patients and their families.
9. Proficiency in documentation and record-keeping.
