Job Title: LPN, Long Term Care (LTC)
Job Summary:
The LPN in Long Term Care (LTC) is responsible for providing compassionate and quality nursing care to residents in a long-term care facility. They work under the supervision of registered nurses and physicians to assess, plan, implement, and evaluate nursing care for residents.
Responsibilities and Duties:
1. Administer medications and treatments to residents as prescribed by healthcare providers.
2. Assist with resident assessments and care planning.
3. Monitor resident health status and report any changes to the healthcare team.
4. Provide wound care, catheter care, and other nursing procedures as needed.
5. Assist residents with activities of daily living, including bathing, dressing, and toileting.
6. Collaborate with other healthcare professionals to provide comprehensive care to residents.
7. Maintain accurate and up-to-date documentation of nursing care provided.
8. Educate residents and their families on health-related topics.
9. Adhere to all facility policies and procedures to ensure the safety and well-being of residents.
10. Participate in staff meetings, in-service training, and quality improvement initiatives.
Qualifications and Skills:
1. Graduation from an accredited practical nursing program.
2. Current LPN license in the state of practice.
3. CPR certification.
4. Prior experience in long-term care or geriatric nursing preferred.
5. Strong interpersonal and communication skills.
6. Ability to work effectively in a team environment.
7. Knowledge of nursing standards of practice and regulations.
8. Compassion and empathy for residents and their families.
9. Strong organizational and time-management skills.
10. Ability to prioritize and multitask in a fast-paced environment.
