Job Title: Long Term Care (LTC) Nurse
Job Summary:
The Long Term Care (LTC) Nurse is responsible for providing high-quality and compassionate care to patients in long-term care facilities. They will work closely with other healthcare professionals to assess patients' needs, develop care plans, administer medications, and monitor patients' progress.
Responsibilities and Duties:
1. Perform assessments and develop individualized care plans for patients.
2. Administer medications and treatments as prescribed by physicians.
3. Monitor patients' vital signs and response to treatment.
4. Provide emotional support and education to patients and their families.
5. Collaborate with interdisciplinary healthcare team members to coordinate patient care.
6. Document patient assessments, care plans, and progress notes accurately.
7. Participate in quality improvement initiatives to enhance patient outcomes.
8. Adhere to facility policies and procedures to ensure patient safety and compliance with regulations.
9. Assist with activities of daily living, such as bathing, dressing, and feeding.
10. Maintain a clean and organized work environment to promote infection control.
Qualifications and Skills:
1. Current RN or LPN license in the state of practice.
2. Minimum of 2 years of experience in long-term care or a related field.
3. Strong clinical assessment and critical thinking skills.
4. Excellent communication and interpersonal skills.
5. Ability to work independently and as part of a team.
6. Proficiency in electronic health record systems.
7. BLS certification.
8. Compassionate and empathetic attitude towards patients.
9. Attention to detail and strong organizational skills.
