Job Title: LPN Primary Care (Licensed Practical Nurse)
Job Summary:
We are seeking a compassionate and dedicated LPN Primary Care to provide exceptional patient care within our healthcare facility. As an LPN Primary Care, you will work collaboratively with a team of healthcare professionals to assist in promoting and restoring patients' health. Your main responsibilities will include providing patient care, monitoring vital signs, administering medications, and maintaining accurate records.
Responsibilities and Duties:
1. Deliver quality patient care, ensuring patients' physical and emotional well-being.
2. Collaborate with physicians and Registered Nurses (RNs) to develop and implement patient care plans.
3. Perform nursing procedures, including wound care, injections, and assisting with minor surgical procedures.
4. Administer medications as prescribed by physicians, ensuring proper dosage and timely administration.
5. Monitor patients' vital signs, documenting changes and reporting any abnormalities to the healthcare team.
6. Assist in admitting and discharging patients, providing education and instructions related to their care.
7. Maintain accurate and detailed patient records, ensuring confidentiality and adherence to legal regulations.
8. Promote infection control measures, including proper sanitation and precautions to minimize the risk of healthcare-associated infections.
9. Collaborate with other healthcare professionals to facilitate a multidisciplinary approach to patient care.
10. Provide emotional support and comfort to patients and their families, ensuring they feel heard, understood, and respected.
11. Stay updated with the latest developments in nursing practices and procedures through continuous professional development.
12. Adhere to ethical and professional standards, demonstrating competency and integrity in all aspects of nursing care.
13. Participate in quality improvement initiatives and contribute to the development of policies and protocols.
14. Maintain appropriate inventory of supplies and equipment, ensuring their availability when required.
15. Uphold and promote a positive and caring work environment that fosters teamwork and collaboration.
Qualifications and Skills:
1. Must possess a current and valid LPN license in the state of practice.
2. Completion of an accredited LPN program.
3. Previous experience in a primary care setting is preferred.
4. Strong knowledge of nursing practices, standards, and procedures.
5. Excellent interpersonal and communication skills, both verbal and written.
6. Ability to work effectively in a fast-paced environment, managing multiple tasks and priorities.
7. Proficient computer skills, including electronic health records systems and basic Microsoft Office applications.
8. Exceptional attention to detail and strong organizational skills.
9. Ability to maintain professionalism and composure while providing care in high-stress situations.
10. Strong critical thinking and problem-solving abilities.
11. BLS (Basic Life Support) certification is required.
