Job Title: Case Manager, Home Health, Hospice
Job Summary:
The Case Manager in the Home Health Hospice setting is responsible for coordinating and managing the care of patients receiving end-of-life care. They work closely with the interdisciplinary healthcare team to ensure patients receive quality care and support during this challenging time.
Responsibilities and Duties:
1. Assess patient needs and develop individualized care plans.
2. Coordinate with physicians, nurses, social workers, and other healthcare professionals to provide comprehensive care.
3. Monitor patient progress and adjust care plans as needed.
4. Provide emotional support and guidance to patients and families.
5. Educate patients and families on end-of-life care options and resources.
6. Ensure compliance with regulations and company policies.
7. Collaborate with community agencies and organizations to support patient care.
8. Maintain accurate and up-to-date patient records.
Qualifications and Skills:
1. Registered Nurse (RN) or Social Worker (MSW) license required.
2. Previous experience working in home health or hospice care preferred.
3. Strong communication and interpersonal skills.
4. Ability to work independently and as part of a team.
5. Compassionate and empathetic demeanor.
6. Knowledge of end-of-life care and resources.
7. Excellent organizational and time management skills.
8. Ability to handle stressful situations with professionalism and composure.
